At Fukuchi Skin Clinic, we also offer an outpatient rehabilitation facility, the ai Day Care Center.

To help people with various physical or mental disorders return to society, we provide medical treatment using exercise therapy methods, occupational therapy, counseling, and more.

Our goal is to improve and maintain mental and physical functions.

How to use this facility

If you have not yet applied for long-term care, please apply directly to our clinic or to a nursing care support provider.

Please use our clinic in accordance with the care utilization plan you have prepared.

Pick-up and drop-off

Pick-up
We leave our clinic at 9:00 a.m. to pick you up directly from home and transport you to our facility (8:30 a.m. for 6-hour day care). From there we provide training and treatment for 4 to 6 hours.

Drop off
We also provide drop off service after our session is complete . We request that family members meet patients at the car upon arrival.

Schedule changes, cancelations, and holidays

Schedule changes can be made at any time by consulting with a care manager. (We may not be able to accommodate your request.)

If you know in advance that you cannot attend a session, please contact us as soon as possible.
If you require same-day cancelation, please contact us between 8:30 and 8:45 a.m.
If you contact us late or cancel without permission, we will charge you the cost of lunch for that day.

Our clinic is closed on public holidays.

Fees

Legal fee: or day-care rehabilitation as stipulated by long-term care insurance
Lunch fee: 500 yen (per lunch box used)
Others: 500 yen for take-out fee (per month of use)

Other fees, such as extension fees (in principle: for basic services only), will be determined upon consultation with the user.

About the correspondence book

Please check the correspondence book when you get home. If you have any messages, please record them here. The days of use are written on the inside back cover.

Treatment and management of chronic illnesses

As a general rule, we ask that you continue to receive treatment for chronic illnesses from your current doctor. If you do not have a doctor, we will refer you to an appropriate medical institution.

Emergency responses and medical care in our specialty areas will be handled at our clinic, so please bring the following with you when coming to the facility.

Health insurance card
Nursing care insurance card: confirmed at the beginning of the month
Elderly medical care recipient card
Elderly medical care fees (legal fees)

Personal information

We will share personal information among relevant parties to make more effective use of the long-term care insurance system.

Other

Please come to the clinic in clothes that are comfortable for exercise. If you need a waist belt, please wrap it at home before coming. You are welcome to visit at any time.

Please be careful not to bring more cash than you need. Please make sure you bring exact change for usage fees, lunch fees, etc.
